Finasteride, a synthetic 4-azasteroid compound, is a specific competitive inhibitor of type II 5-alpha reductase, an intracellular enzyme that converts testosterone to the more active androgen dihydrotestosterone (DHT). In BPH, its increase depends on the conversion of testosterone to DHT in the prostate. Finasteride (Propecia) is very effective in reducing the concentration of DHT in plasma and prostate tissue.

Its increase in BPH depends on the conversion of testosterone to DHT in the prostate. Finasteride is very effective in lowering the concentration of DHT in blood plasma and in prostate tissue. Suppression of DHT formation is accompanied by a decrease in prostate size, a maximum increase in urine output, and a decrease in the severity of symptoms associated with benign prostatic hyperplasia.

Finasteride has no affinity for androgen receptors.

According to the results of clinical trials (PLESS), which involved patients with moderately or severely symptomatic BPH and an enlarged prostate, finasteride reduced the incidence of acute urinary retention by 7/100 to 3/100 over a period of 4 years, and the frequency of need for surgery – transurethral resection of the prostate (TURP) or prostatectomy – 10/100 to 5/100.These changes were also associated with an improvement in BPH symptoms (2 point decrease on the quasi-AUA symptom scale), a consistent decrease in prostate volume of approximately 20%, and a consistent increase in urine output. .

Use of the substance finasteride

Treatment of benign prostatic hyperplasia and prevention of urological complications to reduce the risk of acute urinary retention, to reduce the risk of requiring surgery, including transurethral resection of the prostate and prostatectomy.

Treatment to reduce the size of an enlarged prostate, improve urination and reduce the severity of symptoms associated with benign prostatic hyperplasia.

In combination with doxazosin – to reduce the risk of progression of symptoms associated with benign prostatic hyperplasia.
